Rebate Details:
- To receive your rebate, please complete the form below with all required fields and upload your proof of purchase.
- Offer is $5 off a case, up to 10 cases ($50 max) and is limited to foodservice operators only.
- Products must be purchased by December 31, 2025.
- Offers must be submitted online or postmarked by January 31, 2026.
- Rebate is taken from LIST PRICE and is NOT VALID on any other promotion or discount.
- Receipt/invoice/velocity report must be legible.
- Receipt/invoice/velocity report must contain store name and address, purchase date, and product information.
- Circle or highlight the Rosina items on the receipt/invoice/velocity report you submit.
- FIVE redemption forms per foodservice operation, name, or address.
- Please allow up to 8 weeks for check to be delivered.
Canadian Customers: Rebate check is processed in U.S. funds.
